A client intake form grows every time a project exposes a missing detail. Eventually it asks so much that nobody knows which answers matter. Review the form by giving every question a purpose. If an answer does not change scope, feasibility or the next conversation, it may belong later. The first questionnaire should help both sides understand the work, rather than collect everything that might someday be useful.
Start with the desired change
Ask what the client wants to improve and what happens today. A question such as where does the current booking process break down can reveal more than asking for a list of desired features. Invite a recent example so you can understand the situation without guessing.
Separate a goal from a proposed solution. A client may request a new application when a simpler process would solve the immediate problem. Record both and explore the difference together.
Ask about constraints before details
Find out who can make decisions, which systems must stay and whether there is a real deadline. Ask what has already been agreed internally. These answers shape what can be proposed. Make it acceptable to say that a budget or deadline is still undecided.
Do not ask for passwords or complete customer datasets in the intake form. Describe any later access request separately, using a suitable secure method and only the permissions required.
Give the next conversation a focus
Use the answers to identify three points for discussion. For example, the first usable outcome, a required integration and who will review the work. Mark missing information rather than treating a blank answer as a negative response.
Tell the client why you are asking a less obvious question. Context can make the questionnaire easier to complete and reveal when the wording needs improvement.
Test with one actual enquiry
Try the form on a real, suitable enquiry and ask which question was confusing. Compare the answers with what you needed during the conversation. Remove duplicate questions and move specialist details to the appropriate stage. template.fyi lists a Client Intake Questionnaire among its downloads. Inspect the contents before selecting it. Adapt a format to your work and privacy responsibilities; the finished form should make a useful conversation easier to start.
